I am the parent or legal guardian of the minor participant identified during registration ("Participant"). I understand that participation in the NextGen Futbol Homeschool Soccer Program ("Program"), including soccer training, physical education activities, games, drills, conditioning, stretching, and related activities, involves inherent risks.
